Medical Forms

Located in the Parent Portal and Submitted via Blackbaud by Friday, August 1, 2025.

Download, scan and email to school nurse by Friday, August 1, 2025.

• Health Assessment Record *Please note the immunization record is part of this form. The State of Connecticut requires a complete, up-to-date immunization record for each child, returning and new, before attending the first day of school. For returning students, this form needs to be submitted from the date of your child’s last physical examination.

The following should be scanned and emailed to the nurse if applicable to your student:  

• Asthma Medication Authorization and Asthma Action (Inhaler)

• Anaphylaxis Emergency Medication Authorization & Action Plan (EpiPen)

• Authorization for Administration of Medication (State of Connecticut regulations require a medical provider’s written order and authorization by a parent)
