Health Assessment Record *Please note the immunization record is part of this form. The State of Connecticut requires a complete, up-to-date immunization record for each child, returning and new, before attending the first day of school. For returning students, this form needs to be submitted from the date of your child’s last physical examination.
The following should be scanned and emailed to the nurse if applicable to your student:

Authorization for Administration of Medication (State of Connecticut regulations require a medical provider’s written order and authorization by a parent)

For those new to KO, Shield & Dragon students are those students in Forms 4-6 who have applied for and received placement as student ambassadors to conduct tours with prospective families.

A Daatco bus services Glastonbury and the surrounding areas. KO facilitates and processes the fee associated with the ride. Click on this link to sign up.
West Hartford
The Town of West Hartford operates a bus to and from KO for residents. If you plan on using this bus please complete the West Hartford Bus Transportation Form. This form is Due: July 1 to the Town of West Hartford.
